How to Prepare to Your First Visit
Your first consultation will usually include an assessment and discussion of your treatment choices. Treatment is usually completed or started on the same day as the consultation. However, a complex medical history or treatment plan will necessitate an examination and a second session to deliver therapy on a different day.
Please aid us by supplying the following information during your consultation:
- Any x-rays obtained from a previous dentist, if applicable.
- A list of the medications you are now taking.
- If you have dental insurance, bring the completed forms. This will save time and allow us to assist you with any claims.
Please see that any patient under the age of 18 must be accompanied by a parent or guardian.
Please notify the office if you have a medical condition that requires attention prior to treatment (e.g., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (e.g., heart medications, aspirin, anticoagulant therapy, etc.).
